Indoor Player Registration/ Waiver

New Player Info needed:

Please turn in the following to Rob by JULY 1, 2008:

1. Please fill out both Outdoor forms listed above. You can fill them in on line, then print, sign  and turn in. Forms must be notarized.

2. A copy of your daughter's birth certificate.

3. Two school pictures for player cards- one for outdoor, one for indoor.

4. Fill out indoor form when requested.

5. All the collection/ timing of fee's & forms will be communicated to you as needed. All may be dropped off on Rob's front porch in the collection box, mailed, or delivered at game/ practice.
